Cancel / Freeze Request Form

A written notice via email or this request form is required for membership freezes or cancellations 20 days before your next billing cycle. This will ensure we have ample time to complete the request prior to your next auto-pay deduction. We do not offer refunds if we are not notified in advance. This form is just a request, and your membership is not automatically canceled or frozen just by submitting this form. Once we receive this form, we will notify you via email and confirm your membership has been canceled or frozen per your request. If you have not received a confirmation email confirming your request has been approved, it means your account has not been handled. Membership freezes available in one-month increments with no-charge up to 3-months in a 12-month period. All discounted or special rate memberships will no longer qualify for the discounted rate once membership has been canceled. If you would like to keep your locked-in special, we recommend freezing your membership.
